Because, "way back in the day," many people were required to insist that transition would allow them to live as straight people, and if they didn't, the clinics wouldn't have considered them candidates for transition at all. The gatekeepers didn't see gay trans people as "legitimately" trans or as existing in any significant numbers. But maybe that narrative has flipped on its head in recent years, with more people feeling freer to self-identify their sexuality.
