The person nearest the item (the “queer”) must pick it up and then run to avoid getting tackled (“smeared”) by the rest of the group. Once smeared, the queer throws the item again and play resumes.

What a metaphor—They don't choose queerness. It literally falls out of the sky and lands on their head. Everyone sees the queerness being not at all that kid's fault, yet they tackle the queer anyway.
